1. Please note that if your billing address differs from where you need your order shipped; you will need to click the download button below and fill out a form.

2. Please fill out and Upload, email, or take a picture of the completed form along with valid identification.

3. If you have selected ship to nearest terminal, you will be required to pickup your order from the nearest terminal. The shipping company themselves will call you to let you know when your shipment is ready for pickup. Please insure your phone number is correct. They may also as for goverment issued ID just to confirm who is picking up the shipment :)!

4. Also, purchases via PayPal can only be sent to the registered address supplied via PayPal; so please do ensure that your shipping address is up to date.
